Terms and conditions for March Break Camp 2020 (Alpha Kid and Teen)

Waiver by Parent/Guardian

I understand and acknowledge that Alpha Coding is making its facility available for my child to participate in the event/classes hosted by it, in view of my express release of their liability for any cause whatsoever. I, understand, and agree to declare this release and waiver as under:

I understand that the course program may involve potentially hazardous tools or materials i.e. robotics / electronics course, 3D printing, interactive game involving projection on the wall, other hardware and devices. I release Alpha Coding of any and all liability and waive my rights, against the releasees.

I understand that my child may suffer physical harm as a result of the acts or omissions of me, Alpha Coding, or other participants due to his or her participation in activities at, with, or sponsored or administered by Alpha Coding. My child elects to participate in spite of the risks.

I will pay for my child’s and my own emergency medical expenses and all subsequent medical expenses in the event of any incident, accident, illness or incapacity, regardless of whether I have otherwise authorized such expenses.

If my child has Down Syndrome, he or she has had a neck x-ray and other medically recommended tests to screen for Atlantoaxial Instability, and has been cleared to participate in all activities at, with, or sponsored or administered by Alpha Coding, and I have delivered Alpha Coding a note from my child’s physician confirming the same.

I own responsibility to pick up my child at a scheduled time. In the event child is sick or has certain health problems Alpha Coding shall not be held liable for misshaping.

I agree not to institute any suit or action at law or otherwise against Alpha Coding in Canada or anywhere in the world, or to initiate or assist in the prosecution of any claim for damages or cause of action which my child or I may have by reason of injury to my child, myself, or property related to, arising out of, or in any way connected with my child’s participation in activities at, with, or sponsored or administered by Alpha Coding.

I agree to indemnify, defend, and hold harmless Alpha Coding from any and all losses, liabilities, claims, expenses, actions or proceedings of any kind which may be initiated by myself, including on behalf of my child, or any other person or entity related to, arising out of, or in any way connected with my child’s participation in activities at, with, or sponsored or administered by Alpha Coding. This includes reimbursement for all legal costs and attorneys’ fees incurred by Alpha Coding, myself, and other indemnified parties, or any of them, for the defense of any such actions.

I hereby voluntarily, fully and forever waive, release and discharge Alpha Coding its owners, officers, directors, agents, employees, volunteers, and all other persons or entities acting in any capacity on its behalf from any and all losses, liabilities, claims, expenses, demands, actions, damages, injuries, causes of action, and rights of action which are related to, arise out of, or are in any way connected with my child’s participation in activities at, with, or sponsored or administered by Alpha Coding. The foregoing waiver of liability includes, without limitation, damages or injury resulting from the negligence of Alpha Coding (whether such claims are based upon breach of contract, breach of warranty, or any other legal theory) or from any other cause or causes.

I hereby consent and agree that Alpha Coding may use child’s photos or excerpts taken from the video system for purposes of publicity. It is further expressively agreed that this release and waiver is intended to be as broad and inclusive as submitted by the laws of the Province of Ontario, and it is hereby further agreed that the effects of this release and waiver shall continue to operate after the termination of the contract of membership which exist between the parties hereto.

Terms and conditions for Participants

Beverages and food:

• Alpha Coding does not provide food, snacks, nor beverages.
• If you bring food/snack, it must be nut-free.

Cancellation:

• $100 cancellation fee applies if cancelled less than 5 days prior to the event
• If there are less than 5 participants, the March break camp will be cancelled and the fee will be fully refunded

Refunds:

• Refunds will be pro-rated
• $60 administration fee applies

Damages to equipment and facility:

• Damages to equipment (laptops, tablets, 3D printer, etc.) and the facility as a result of negligent and/or deliberate act of participant are the responsibility of the guardian/parent of the participant.

Additional forms:

• You agree to fill out and sign a mandatory Health Form and Sign Out Form prior to or on the first day of class at our learning centre.

Authorized person to pickup minors:

• Parents/guardians understand and agree that it is mandatory to show government-issued ID to Alpha Coding staff member at every pick-up to confirm identify, except for self sign out students.
